Medical Surveillance Questionnaire

EHS MSQ Transition from eRAM to Enterprise Health Portal

Beginning December 7:  

  • eRAM will no longer send out any MSQ email notifications.
  • No one should fill out the MSQ form in eRAM.
  • Protocol Animal Handlers will start completing the medical surveillance form in the U-M Enterprise Health (EH) portal, and EH will send out the reminder email notifications.
    • Note: Non-Protocol Animal Handlers (ULAM and ACUO staff) have already been completing the MSQ forms in Enterprise Health (and getting emails from EH) since 2024.

eRAM will be updated daily to reflect MSQ requests and completions. PI/Lab Personnel and the ACUO can view status via the ‘OHSP’ tab on Protocol Workspaces.

Additional Information

Effective December 7, 2025, the Environment, Health & Safety Medical Surveillance Form (EHS Med Surv Form) for protocol personnel will be completed within the U-M Enterprise Health (EH) portal instead of within the eRAM system.

Individuals who are due to complete their annual Animal Handler Medical Surveillance Questionnaire with OHS will receive an email from [email protected] approximately one month (30 days) before their requirements are due. They should follow the ‘Employee Action Items’ in the email and complete the questionnaire via the Enterprise Health portal, where an OHS provider will review it. 

Some Animal Handlers may need to schedule medical surveillance visits at OHS based on their animal contact or health status. 

Upon receiving proper clearance from OHS, employees and their supervisors will be notified via an EH portal message with clearance date and next due date. 

Note, new researchers will be notified via email from [email protected] explaining they are part of the Annual Animal Handler Medical Surveillance program.

You can access the Enterprise Health portal with your U-M Uniqname login, password, and two-factor authentication. The auto-generated “Medical Surveillance Requirements Due” reminder email also includes a link to the portal.
